Daily Home Screening
Please check your child for the following symptoms daily. If your child has any of the listed symptoms, this could indicate a possible illness that may decrease the child’s ability to learn and also put them at risk for spreading illness to others.
*If your child currently has any of the following symptoms, not caused by another heath condition, please contact the school nurse, BEFORE sending your child to school.

Temperature of 100.4° or higher

Headache

Shortness of breath or trouble breathing

Runny nose or congestion

Sore throat

Cough
-
Nausea, vomiting, diarrhea, or abdominal pain

Body aches, chills, fatigue

New loss of taste or smell

**To help prevent the spread of illness, and to ensure the health and safety for all, your child must stay home until fever-free and symptom-free for 24 hours without the use of medication. If you have questions, please contact your school nurse.**
